Pfizer Information

BOOK ONLINE for all three locations.

1st or 2nd Pfizer vaccination for ages 12 and older

6 MONTH BOOSTER for People Age 65 and older, residents in long-term care settings, and people age 50-64 with underlying medical conditions. Click here for a list of those conditions.

Johnson & Johnson Information

Johnson & Johnson is available on Wed 10am-4pm at our Poplar office by appointment only. Please call to book your appointment, online booking is not available. Appointment schedulers are not available on Sat, so call during office hours mon-fri 9a-5p to book your J&J appointment. J&J is one and done vaccine.

Moderna Information

2nd Moderna Vaccinations are available by appointment at our Cordova location every Wed 10am-4pm. Please call to book your appointment, online booking is not available. Appointment schedulers are not available on Sat, so call during office hours mon-fri 9a-5p to book your Moderna Vaccine.

You may not take the vaccine if you are *Allergic to any component of the vaccine or have experience an allergic reaction to reaction to vaccines in the past.
