What is a Titer Test and Should You Get One?

If you’ve ever been asked to prove your vaccine immunity for a new job, a nursing program, or a clinical rotation, you may have come across the term “titer test.” We promise, it sounds more complicated than it is! What Is a Titer Test?

A titer test (sometimes called an antibody titer test or serum titer test) is a simple blood test that measures the level of specific antibodies in your bloodstream. Those antibody levels indicate whether your immune system has enough protection against a particular disease.

Antibodies are proteins your immune system produces in response to a vaccine or a past infection. When your titer result comes back with sufficient antibody levels, it generally means you have immunity to that disease. When it comes back negative or below the threshold, it may mean you need a vaccine or a booster.

Titer testing is not a diagnostic test for active illness. It is specifically used to assess immunity status.

Why Would Someone Need a Titer Test?

The most common reason people seek titer testing is a requirement from a school, employer, or healthcare facility. Vaccination records from childhood get lost, families move, paper documents get damaged. When you cannot produce an immunization record, a titer test is one way to demonstrate proof of immunity. These are the most common situations where you would need a titer test:

  • Nursing and healthcare students: Most nursing programs, medical schools, and allied health programs require students to demonstrate immunity to specific diseases before beginning clinical rotations. Vaccination records alone are often not enough, many programs specifically require quantitative IgG titer results.
  • Healthcare workers: Hospitals and healthcare facilities routinely require staff to verify immunity as a condition of employment. The Centers for Disease Control and Prevention (CDC) recommends that healthcare facilities implement vaccination and prevaccination testing policies to protect patients and staff from preventable infections.
  • People who have lost their vaccination records: This is more common than most people expect. If you were vaccinated as a child but no longer have documentation, a positive titer result can serve as an acceptable alternative in many institutional settings.
  • Travelers: Some international destinations or travel health requirements call for verification of immunity to certain diseases.
  • Anyone who wants to know their immunity status: You do not need an institutional requirement to benefit from knowing whether you are protected.

What Does a Titer Test Check For?

A titer test can be ordered for many different diseases. At The Shot Nurse, we offer titer testing for three of the most commonly required:

Chickenpox (Varicella)

The varicella titer is an IgG blood test that checks for immunity to the varicella-zoster virus (VZV), the virus that causes chickenpox. It is one of the most frequently required titers for healthcare students and workers, since chickenpox spreads easily and can cause serious complications in vulnerable patients.

Hepatitis B

The hepatitis B titer measures surface antibody levels (anti-HBs IgG) to confirm immunity to hepatitis B virus. This titer is standard for healthcare clearance, and the CDC provides specific guidance on interpreting results and next steps for healthcare workers whose results come back negative.

MMR (Measles, Mumps, and Rubella)

The MMR titer checks IgG antibody levels for all three viruses. It is commonly required by schools, nursing programs, and healthcare institutions to verify that individuals are protected before entering settings where exposure risk may be higher.

These three titer tests are frequently requested together, and together they cover the immunity documentation most commonly required for school, clinical, and employment purposes.

How Does the Titer Test Process Work?

The process is straightforward. A blood sample is drawn, and the sample is sent to a certified laboratory where antibody levels are measured. Results are typically returned within a few business days, though timing can vary by lab. Your results will generally fall into one of two categories:

  • Positive (Immune): Your antibody levels are above the threshold for immunity. This result can typically be used as documentation for school or employment requirements.
  • Negative or equivocal (Not immune): Your antibody levels are below the threshold, meaning you may not have adequate protection. Depending on the disease and the institution’s requirements, you may need to be vaccinated or receive a booster, then potentially retest to confirm your immune response.

It is worth noting that some institutions require quantitative titers, which provide an actual numerical antibody value, rather than qualitative results that simply read “immune” or “non-immune.” If you are getting tested for a specific program or employer, confirm which type of result they need before you test.
